Before every tattoo and piercing we need your written consent – as required by the Hamburg hygiene rules. Download the form in advance, fill it in at your own pace and bring it to your appointment.

How it works

    The form for a tattoo or piercing – in German or English.

  2. Fill in

    Directly in the PDF on your phone or computer – or printed out with a pen.

  3. Sign in the studio

    Please do not sign yet: we go through the procedure and risks with you, check your ID and you sign with us.

Under 18?

Minors need the consent of their legal guardians. A parent comes to the appointment with ID and signs in the studio.

  • Piercing 16–17all piercings except intimate piercings, with a guardian
  • Piercing under 16lobe piercings only, with a guardian
  • Tattoo under 18only after talking to the studio

Privacy: your details stay with you until the appointment – we only receive the form in the studio and keep it locked away for 10 years. Privacy policy
